It’s easy to notice in photographs, especially selfies, that small pocket of fat that can be so annoying. Up to recently, the only effective treatment for this problem was liposuction. While I’ve performed liposuction successfully for twenty-five years, having a non-surgical in-office option now is fantastic.

Kybella® is an FDA-approved nonsurgical treatment that contours and improves the submental fullness of a double chin. With a 15 minute in-office treatment, it destroys fat cells in the specific areas it is injected.

I provide these treatments at our office in Middletown, NY. Treatments vary, but most of my patients undergo one or two sessions, with about half of my patients being satisfied with one session. The first session generally costs about $1,500, and the second, about $1,200. All the patients shown on this page were treated by me with Kybella®

Kybella® is not for everyone. If you have a wider area of excess fat or a large amount of loose skin, you may be better served with a surgical option. We can go over this during a free consultation. Keep in mind, the results from Kybella®, like CoolSculpting®, liposuction, or any other cosmetic procedure, depend a great deal upon the aesthetic sense and judgement of the physician evaluating you and performing your injections. Our familiarity with the anatomy of the area and our experience with the goals and subtleties of neck liposuction have prepared us well to optimize your Kybella® treatment.
Is it safe? In the right hands, with good patient selection, it is both safe and effective. In fact, I’ve treated myself with it! Here’s how it went:

I used only an ice pack to chill the submental area before the injections. Injecting yourself is usually more painful than being injected by someone else, but I barely noticed the injections. Immediately afterwards, I felt an irritating sort of burning discomfort and tingly numbness on the neck skin and the base of my chin. The area swelled to about twice its original size. I went to dinner and no one noticed anything. I felt the discomfort most when tilting my head back. (Don’t have the treatment right before going to the planetarium.)

Twelve hours later (the next morning) there was no real discomfort. I still had numbness on the chin and neck skin, which was an odd sensation while shaving. My swelling was slightly more (3x original size), such that my wife noticed I was in fact, getting a middle age neck, and asked if I was going to go ahead with that Kybella® treatment I had been talking about. By the next day, the swelling was down to about 50% above normal, and no one in the office commented or suspected I had treated myself. The tingling numb feeling was nearly resolved on the chin and decreasing on the neck skin. So, within 48 hours, the swelling was down to what I would call mild, and the other symptoms hardly noticeable.

My submental neck fat was not that large to begin with, and a larger fat pad could remain swollen for a longer period. But all in all, I consider it a very easy treatment to undergo.

Liquid lifts are treatments we use to rejuvenate or enhance facial features while avoiding a pulled, artificial look. A liquid eye lift is an in-office treatment to rejuvenate the lower eyelids without surgery.

Traditional lower eyelid surgery reduces the bags but involves the risks and downtime of surgery. It can also leave a flattened look to the eyes. Instead of surgery, I now carefully place long-lasting filler around the grooves of the lower lids, which can provide a more youthful, natural-appearing look. The treatment is done in the office and requires no anesthesia.

During your consultation, I use computer imaging to review the goals of a liquid eye lift, as well as explain other possible nonsurgical options, such as a liquid cheek lift, Botox® injections, or filler injections in other areas of the face.

By replacing the lost volume of the central face, especially around the eyes, we can restore a more natural appearance of youth while improving distracting signs of old age, such as grooves and bags beneath the eyes. This is often combined with more traditional use of fillers around the mouth to soften fine lines.

The lip enlargements are done with a cannula for safety and comfort. In the FDA study, most patients still felt they had a significant lip enlargement at one year following injection. The amount of filler used can be adjusted in 0.5cc increments according to your goals (1.0cc, 1.5cc, 2.0cc).

Nonsurgical Facelift Tip Sheet

A “showcase” minilift result by Dr. Loomis (not a guarantee of results)

A nonsurgical facelift, a minilift, “liquid lift,” and other in-office services can be excellent ways to look better without resorting to major surgery. But with so many options out there, how do you know which option, if any, is right for you? During a consultation, before going over pros and cons of various procedures, I first help the patient honestly and accurately identify a realistic goal. This is something you can start on your own at home.

Here are five questions I suggest you ask yourself before making a consultation for a nonsurgical facelift.

What bothers me the most? Don’t say, “everything,” even if it’s true. For a few days, try to nail down the one thing that bothers you the most. What do you find yourself noticing more than anything else? Frown lines? A fold of skin by your mouth? A tired look to your eyes? Make yourself pick one thing, then write it down. After that, write down whatever came in second place.
What am I hoping to achieve? Be honest. If you want every wrinkle on your face erased, think about it. Even with aggressive surgery this is not possible. So, short of that, what are you hoping for? Try to show your goal in the mirror by pushing, lifting or pulling. Then estimate percentages of that goal: 80%, 50%, 30%. Now, pick the lowest of those percentages that would still make you happy. Have a friend photograph it.
What downtime can I handle? Any treatment that introduces energy into tissue, can cause swelling. Any injection can cause bruising. So even without surgery there can be downtime. Worst case scenario: what degree of swelling or bruising can you accept and for how long? Write it down.
What risk am I willing to accept? While nothing in life is totally “risk-free,” there are gradations of risk. The risk of general anesthesia is greater than that of local anesthesia. Are you willing to accept that increased risk, or do you want to limit your options to treatments under local or no anesthesia?
Who will I ask to do this? Now that you are clear about what you are looking for, find a doctor who is board certified by the American Board of Medical Specialties, and experienced in facial rejuvenation surgery. If you are choosing between a nonsurgical facelift and a surgical one, a doctor experienced with both can give you a perspective from both. Their before & afters should look great to you–remember, those are their best results. See more than one doctor. Tell them your answers to the above questions and ask to see an average, not showcase result. It is the average result, after all, that you are most likely to achieve. Does the average result look good to you? Show the photo from question #2 to the doctor, and ask how likely it is that you can obtain a similar result with the procedure you are considering. If it’s not that likely, you either have to re-evaluate your goals or consider other options.

Being totally honest with yourself and your surgeon ahead of time can help you choose the procedure that’s right for you.

We have two CoolSculpting machines which means we can DualSculpt. That means we can treat two areas at one time, reducing the amount of time it takes to treat multiple areas. We generally expect a 25% reduction in the fat of an area treated. Unlike other methods of fat removal, CoolSculpting involves no surgery, no physical removal of fat, no burning, no wound healing, yet fat is reduced. How?

Using equipment developed by Harvard scientists at Massachusetts General Hospital, fat tissue is cooled to the precise temperature that will damage fat cells, but not other surrounding cells such as nerve or muscle. These damaged fat cells are then removed from the body by a natural process of remodeling which goes on for the next three to four months.

Unlike liposuction and other nonsurgical fat reduction treatments that use lasers or ultrasound to break open fat cells there’s no burning, dead tissue, or open space left behind by CoolSculpting. The cells are taken away by the body, one by one, as the site is remodeled. This is akin to a tadpole’s tale being absorbed as it turns into a frog. The scientific term is programmed cell death. Since it is gradual and natural, the results are smooth and soft. In some areas, such as the bra folds, Dr. Loomis believes his CoolSculpting results rival or exceed his results using liposuction.

We CoolSculpt the thighs as well, inner and outer, with the same goals as elsewhere: to achieve liposuction type results. As with any medical tool, results depend a great deal on how CoolSculpting is utilized. Results vary between patients and no one can guarantee a result, but we do everything we can to make the odds of a good result better. Dr. Loomis assesses every patient to determine if you are a good candidate, taking into account where your fat is, your skin ton and laxity, etc. He also uses computer imaging to demonstrate a realistic goal to you, to be sure you and he are on the same page regarding results. Then, how and where the applicators are placed are critical. We take the time to make sure your treatment is optimized.

After you are marked, the applicators are applied. Some are suction and others lay flat against you. We position you and wrap you to optimize the treatment. Some applicators are on for one hour, others for one and a quarter. With our DualSculpting, we can treat your inner and outer thighs at the same time.

While you are being treated, you can use our wifi hot spot to check your emails, watch a movie, read a book, get other work done, or just take a nap. Then we remove the applicator, massage the site briefly, and go one to the other sites that you are treating that day. After your treatment, you can resume your normal activities.

There is discomfort in the site for the first week or so, such that it will hurt if you bump the area. After a few days, many will feel an itching beneath the skin. Both of these sensations are relieved with mild compression from ace wraps or a Spanx® garment. About 5% of our patients will have more significant pain temporarily, which we control with a topical anesthetic. Unlike surgery, we do not use pain medication after CoolSculpting.

At first, the areas will swell. Then, by around three to four weeks, you will be back to your starting point. Some patients start to see results by this time. We take pictures at three months, by which time you have about 90% of your final result. The contour continues to improve over the next year.

The results appear very gradually. So gradually, that many patients wonder if they’ve had much of a result. Some of the patients shown on this page actually felt this way, until they saw their results side by side.

What does it cost? The photos show a good estimate for the different areas. We have discounts for multiple site treatments and also discount any second treatments of areas. Most of our patients only have one treatment session. When a second session is opted for, it is usually only for a few sites that have some residual fat. In my practice, even multiple treatments with CoolSculpting end up costing less than my surgical treatments would.

Consider also, that what you are paying for is clinical judgement and experience, not just fat freezing. Cost is an important, practical concern, but before undergoing any medical procedure, ask some questions. Find out who the doctor is, are they the one who will evaluate you? Are they board certified? In what specialty?

Minilift and Liquid Lift

A Minilift and liquid lift are two of our most popular procedures to combat the signs of aging. A minilift can restore a more attractive neck and jawline. A liquid lift, using Kybella®, Voluma®, Juvederm®, and Botox®, can revive youthful fullness around the eyes and cheeks, and improve the contour of double chin. With twenty-five years’ experience using traditional surgery to achieve these goals, I now specialize in nonsurgical and minimally invasive procedures to rejuvenate the face without major surgery. Voluma is a longer lasting hyaluronic acid filler from Juvederm®, lasting two years in the FDA study. Dr. Loomis especially likes using it to correct that groove that forms in the middle of the cheek and runs up to the lower lids, as in the patients above.

There are other areas where we want less fullness, like in the submental or “double chin” area. While we can still do syringe fat removal under local anesthesia when necessary, we now have Kybella® to remove this fat without any surgery as in the patients shown below.

Voluma® Liquid Lifts are our preferred treatment to correct age-related volume loss around the eyes, midface and cheek. Voluma® is a long-lasting filler designed for deeper injections in the face. Rather than fill lines, it lifts and restores youthful fullness.

Voluma® addresses some aging concerns that simple tightening of the skin does not help, such as grooves and hollows. It can provide a more natural rejuvenation to the face, avoiding some of the pulled looks we sometimes see after traditional surgical lifts. I use it around the eyes to provide a Liquid Eye Lift.

Early in the aging process, many women begin to notice a groove in the front of the cheek that runs up towards the under eye. This groove, together with a hollowness beneath the cheekbone, is a subtle but powerful sign of aging. After having Voluma® injections, that sign of aging is lessened. Many Voluma® patients will also often have Juvederm® injections around the mouth to soften their laugh lines or lift the corners of the mouth. Juvederm® can also smooth out lip lines and early jowls. Youthful rejuvenation without an artificial, plastic look–isn’t that our ideal goal?

What about safety? Safety is a big concern with all fillers, but especially with a filler used anywhere near the eyes. I prefer a cannula when injecting deeply on the face, both for accurate placement and safety. With gentle, precise placement, most patients note minimal to no discomfort during the procedure. Next to safety, the aesthetics of the injection are the most important factor. I spend as much time analyzing your facial contours as I do injecting the Voluma®. As I recommend with all cosmetic procedures, check out your injector’s certification. Also evaluate their artistic and aesthetic eye, by reviewing their previous results.
